The best outcomes for treatment and recovery happen when movement from one treatment setting or provider to the next is carefully planned and well-coordinated.

Smooth transitions and coordinated, family-driven care planning is a mantra at the DIY Advocacy Center!  When families are informed about what person-centered care plans look like, we can be clear in advocating this approach with those providing services to our children.
